On your first visit, please arrive at the office 15 minutes early so that you can fill out our registration and information forms. Please bring any special insurance forms and your insurance card with you at the time of your visit. If your insurance company requires a referral, you must bring it to your appointment unless one has been forwarded by your primary care physician.

Fees, Insurance and Payment:
We participate in many major health plans and our office will file your insurance claim as a courtesy. Please keep in mind that your insurance company and you are responsible for any remaining balance. We cannot guarantee that your particular policy will cover the service you require.
Most insurance plans include expenses which must be paid by the patient. If your plan requires a co-payment, we ask that this be paid at the time of your appointment. You will receive a monthly statement and we ask you to remit the balance due or call our office at (316) 685-6222 to arrange a payment plan.
